1. Where did you move to?   We moved to 901 E. 2nd Ave, Ste 308 at the NW corner of Arthur and 2nd in the Gateway 6 building, just east of downtown.  This location will be approximately 15 min closer to downtown from our current location.

2.  Will you continue to take my insurance?  We will continue to bill you or your insurance company as we have been for the foreseeable future.

 

3.  Are your phone and fax number going to change?  Our office phone number will remain the same.  Our FAX number has changed to 509-408-0891 and is the current one to use for sending orders, medical records, or other requests for information.
